The Hamilton Family Health Team (HFHT) offers a team based approach to your health care by bringing together different health care providers to co-ordinate the highest possible quality of care for patients of HFHT practices. You have access to these providers right in your family doctor's office. They include:
- Certified Respiratory Educators (CREs)
- Mental Health Counsellors
- Nurses (RNs, NPs, RPNs)
- Pharmacists
- Psychiatrists
- Physiotherapists
- Physician Assistants
- Registered Dietitians
We also offer a wide range of groups and workshops to help you better manage your health.
