If you're looking for a new family doctor, please check the list of Hamilton Family Health Team doctors to see who is accepting new patients right now. You can find out more about each of our physicians and contact their office directly. You can also visit Health Care Connect (1-800-445-1822). This is a program that helps you find a doctor or nurse practitioner if you don't have one. You can also use the program to change family health care providers, but only after you leave your current family health care provider and are no longer on their patient list.
