Technical Support Specialist – Full-time (1.0 FTE)

Your Opportunity:

We are seeking a highly skilled Microsoft 365 Tenancy Specialist to join our healthcare IT team. This role is ideal for someone with deep technical expertise in managing Microsoft 365 environments, including Exchange Online, Teams, SharePoint, Azure AD, and Intune.

As the go-to for all things Microsoft cloud, you will plan and execute the configuration, security, and optimization of our Microsoft 365 tenant, ensuring it supports both our clinical operations and compliance requirements. Quality Improvement Decision Support Specialist – TFT

*Temporary Full-time for a period of up to 12-months with an opportunity to renew based on funding.

Your Opportunity:

The Quality Improvement Decision Support Specialist (QIDSS) is responsible for supporting non-CHC and non-FHT primary care practices across the Greater Hamilton Health Network (GHHN) Primary Care Network (PCN), which includes Hamilton, Haldimand, and Niagara Northwest. Facilitates the implementation of PCN digital health initiatives (with minimal administrative burden placed on the practice), provides consultation to practices on quality improvement initiatives, and engaging teams in the use of data to support quality improvement goals and clinical process change.

Roles and Responsibilities:

  • Collaborate closely with teams engaged in quality improvement to identify, performance measurement information and data quality needs,
  • Support the development and implementation of quality improvement plans,

Senior Financial Planning & Analysis (FP&A) Analyst

Your Opportunity:

The Senior FP&A Analyst plays a critical role in driving performance monitoring and strategic planning for the organization, along with financial stewardship of the new ERP system. Reporting to the Manager, Finance & Decision Support Analytics, the incumbent will support enterprise-wide financial planning activities and lead the design and delivery of management reporting to internal leadership and external stakeholders, including Ontario Health. This position is integral to enhancing financial transparency, enabling informed decision-making, and maintaining data integrity across all finance-related functions.

Roles and Responsibilities:

  • Lead the full financial planning cycle, including multi-year/rolling forecasts, annual operating budgets, and rolling forecasts for clinical and operational programs.
  • Partner with Clinical ops leaders to translate clinical priorities into aligned financial plans and resource allocations.
  • Conduct scenario planning and sensitivity analyses to evaluate the financial impact of strategic and operational decisions.
  • Deliver comprehensive monthly and quarterly reporting packages with insightful commentary and actionable recommendations for senior leadership.

Triage Clinician Full-time (1.0 FTE)

Your Opportunity:

The Triage and Float Clinician is responsible for assessing and prioritizing patient needs within the primary care centralized hub(s). This role ensures timely access to appropriate care by evaluating symptoms and functioning, assessing social determinants of health, determining urgency, and directing patients to the most suitable healthcare services. The clinician will collaborate closely with primary care providers, interdisciplinary healthcare providers and community resources to ensure seamless patient care.

Roles and Responsibilities:

  • Patient Assessment & Triage: Conduct brief initial patient assessments via phone, virtual consultations, or in-person to determine the level of urgency and appropriate care pathway.
  • Clinical Decision-Making: Utilize clinical judgment and evidence-based guidelines to triage patients to primary care providers, secondary care, tertiary care, community services, emergency services, interdisciplinary team-based care services or other specialized care and programs.
  • Care Coordination: Work closely with HFHT physicians, NPs, PAs, nurses, mental health counselors, social workers, registered dietitians, pharmacists, physiotherapists and psychiatrists while also coordinating with our community and health care partners to facilitate timely referrals and follow-up care.
  • Brief Intervention and Case Management: Provide brief, solution-focused interventions and carry a small patient caseload as patients are connected to the appropriate resources.
  • Crisis Intervention: Identify and manage urgent cases, providing de-escalation support and appropriate crisis referrals.
  • Patient Education: Provide patients and families with guidance on symptom management, self-care strategies, and when to seek further medical attention.
  • Documentation & Compliance: Maintain accurate and timely documentation in electronic medical records (EMRs), adhering to regulatory and organizational standards.
  • Quality Improvement: Participate in ongoing training, case reviews, track and manage data and process improvement initiatives to enhance triage efficiency and patient care outcomes.
  • Float: May be required to carry a small patient case load or back-up mental health coverage if capacity allows.

Junior Accounting Analyst – Temporary Full-Time (1.0 FTE)

Your Opportunity:

The Junior Accounting Analyst is responsible for supporting daily financial operations including accounts payable, accounts receivable, and month-end reconciliation activities. This role ensures timely vendor payment processing and accurate receivables billing, while maintaining data integrity within Sage 50. The ideal candidate demonstrates strong accounting acumen, attention to detail, and the ability to operate effectively in a fast-paced, deadline-driven environment.

Roles and Responsibilities:

  • Process vendor invoices, employee expense claims, and credit card transactions in accordance with internal policies and deadlines.
  • Prepare and issue timely remittance advice and ensure reconciliation of vendor accounts.
  • Liaise with internal departments and external vendors to resolve invoice discrepancies and manage follow-ups.
  • Support the implementation and enforcement of internal controls for AP processes.
  • Generate and submit invoices for receivables in Sage 50 based on program contracts and service agreements.
  • Track AR and AP aging, follow up on outstanding balances, and reconcile customer accounts.
  • Maintain vendor records and ensure accurate GL coding.
  • Assist with the month-end and quarter-end close processes by reconciling AP and AR subledger accounts.
  • Contribute to the development and documentation of AP-related financial policies and SOPs.
  • Support the audit process by preparing documentation and schedules for external auditors.
  • Lead resolution of AP-related audit findings and contribute to ensuring the financial statements are audit-ready by the end of Q3.
  • Collaborate with the Financial Accountant to ensure timely and accurate recording of liabilities.

Qualifications and Skills:

  • Bachelor’s degree in Accounting, Finance, or related field.
  • Minimum 1–3 years of AP or general accounting experience.
  • Experience with accounting/ERP systems (Sage Intacct preferred).
  • Strong organizational skills and attention to detail.

Seeking a dedicated and skilled Physician Assistant to join our Family Medicine team on a part-time basis. The Hamilton Family Health Team is looking for Physicians to join our Medical Assistance in Dying Community Outreach Team.

If you are compassionate about providing end-of-life options to the residents of Hamilton and the surrounding area, you may be interested in joining our team. We are a group of professionals that help individuals reach their end-of-life goals by providing home visits for assessments and provisions of MAiD. Our team works within the recommendations of CAMAP (Canadian Association of MAiD Assessors and Providers) as far as medication protocols and standards of care.
