Escarpment Health Centre is a part of the Hamilton Family Health Team and a full service, large multi-doctor family practice on the West Hamilton Mountain. Our inter-professional team includes family physicians, nurses, dietitians, pharmacists, mental health workers, community coordinators, home care, respiratory therapists and administrative support. The registered practical nurse (RPN) or Registered (RN) is responsible for operating within their full scope of practice, working collaboratively with all members of the Health Centre in providing direct primary care, delivering comprehensive, high-quality patient care, and participating in the development, implementation and promotion of health and prevention of disease.
Duties of the RPN/RN will be divided into three categories: clinical duties, health care team responsibilities and maintenance of the practice environment.

Overview of Role

The start date for this role is January 5, 2026. This is 1-year contract position ending on January 5, 2027.

  • Assist the MD and team with patients’ health needs, health promotion and disease prevention.
  • Initiate the Electronic Medical Record (EMR) of the patient visit, purpose, history and health care needs. Record patient vitals and prepare patient and room for examination – including, routine blood pressure and weight checks.
  • Maintain the EMR (Telus/PS Suite) to recall patients for health promotion and chronic disease management (ex. diabetes, hypertension), well-baby/child care, and disease prevention follow-up such as immunizations, pap smears, mammograms, FIT testing, bone density and diabetes care.
  • Administer and record immunizations. Educate patients of possible side effects.
  • Perform blood pressure follow-ups that include assessing the patient’s response to medications, providing patient education and self-management strategies as per the HFHT program guidelines.
  • Perform staple removal, wound care, ear syringe, urine dips and assist MD with minor clinic procedures.
  • Review and manage incoming prescriptions and renewals with the MD.
  • Scan faxes into EMR/data entry
  • Triage patient phone calls
  • Maintain accurate, concise and confidential EMR documentation pertaining to patient history and encounter data, while upholding patient safety and confidentiality at all times.
  • Maintain immunization inventory and accurate refrigeration temperatures.
  • Maintain patient confidentiality and work in a manner that ensures patient safety.

This team member is responsible for greeting patients, escorting them to exam rooms, and preparing them for their appointments.
