*The Hamilton Family Health Team is working with Health Care Connect and Ontario Health as part of Ontario’s plan to connect every person in the province to primary care by 2029. Registering with Health Care Connect ensures you will be connected with a primary care provider, and is a part of our attachment process. If you have already previously signed up with Health Care Connect, you can skip right to Step 2.
