Nurses within the Hamilton Family Health Team work with your doctor and team members to provide care that promotes health, manages chronic illness and prevents disease.
Appointments
You may request an appointment with a nurse by calling your family doctor’s office. All visits are covered by the Ontario Ministry of Health for patients of the Hamilton Family Health Team.
What do nurses do?
Nurses in each doctor's office are a key resource for their patients. They have the skills and expertise to help you meet your health care needs, including physicals, well-baby visits, diabetes care and immunizations. Nurses can answer your health questions, provide education, and link you to practice and community supports. They are the front line of the primary care team dedicated to your health and well-being.
There are different types of nurses:
- Registered Nurse (RN)
- Registered Practical Nurse (RPN)
- Nurse Practitioner (NP)
