Pharmacists in the Hamilton Family Health Team (HFHT) work directly in family doctors' offices so that you can talk to them one on one. Pharmacists work with you and your doctor to get you well and keep you healthy. They work with your family doctor and with you as part of your health care team to choose the best medication for you.
Appointments
You may request an appointment with a pharmacist by calling your family doctor’s office. All visits are covered by the Ontario Ministry of Health for patients of the Hamilton Family Health Team.
What does a pharmacist in a doctor’s office do?
Pharmacists help you to manage your medication and make sure that your medications are working well for you. They perform medication reviews and help you manage chronic conditions. The first visit usually talks about:
- prescribed medicine
- over-the-counter medication
- natural/herbal medicines
