The Quality Improvement Plan (QIP) plays a key role in the HFHT's efforts to achieve our vision of "a healthier Hamilton". The QIP outlines ways in which we are working to provide care that is safe, timely, equitable, patient-centred, effective and efficient.
The goals of our QIP include:
- Ensuring a safe environment for patients and health care providers
- Improving access to the care patients need when they need it
- Helping patients stay healthy through regular screening
- Helping patients better manage chronic illnesses such as diabetes
- Offering patients opportunities to provide feedback about their experiences and input into how to improve care
- Partnering with other health care providers and community organizations to increase health care system efficiency and co-ordination
The QIP consists of three parts:
- Progress Report: The progress the HFHT has made on the targets we proposed in the previous year's QIP and what we learned in the process
- Work Plan: What we will measure/track, our targets and the changes we plan to make over the coming year
- Narrative: A description of some key activities the HFHT is involved in beyond the work plan
