It Takes a Village!
To create our latest Strategic Plan, we went through an extensive review and engagement process, including the following steps:
- Assessment of the results of our previous Strategic Plan.
- Environmental scan of the primary care environment in Hamilton and beyond.
- Interviews with external stakeholders (i.e., senior leadership of partner organizations).
- Multiple consultations with internal stakeholders.
- Conducted a patient survey and focus groups.
- Oversight, support, refinement, and approval of the Plan by HFHT’s Board of Directors and an executive committee of the Board.
